It wasn’t too long ago that there was a Facebook name land grab for personal usernames as well as fan page usernames. When it was first announced, there were a few restrictions on which pages were eligible for the custom URLs. Now, you only need to have more than 25 fans.
To get started, just go over to http://www.facebook.com/username, and you should see the following.

If you haven’t already set up your own custom URL, you can do that from here. If you have, click on Set a username to get started.

Select the page you want to set up a username for. It will check the availability of the current name of the page and offer that name to you if it is available. If it isn’t or you want to name it something else, you are free to do so. Well, that’s it. Go get your custom URL and share it in the comments.
